Consultation for At-Home Ketamine Treatment
During this 90 minute appointment you will meet with the provider for an evaluation to determine your eligibility and discuss at-home ketamine treatment options.

Once you request to schedule your consult, paperwork will be sent to you for you to complete. After review of paperwork, if you qualify your booking will be confirmed. If you do not qualify you will receive a full refund and an explanation as to why you are not eligible.

I am only able to take Wellmark insurance plans at the moment, but I will be accepting more plans in the near future. Until then, I’m offering flexible, affordable options so you can get timely, high-quality integrative mental health care without waiting.

Returning patients
You have choices:
Follow-Up Visit:
30 minute appointments, focused medication management, supplement suggestions, behavioral management systems, symptom review, and plan adjustments in a single visit.
Schedule as often as you like (at least every 6 months is required for those with prescription medications.
$149 per visit (cash option) (In-person or Telehealth)

Telehealth Subscription: $59/month (Telehealth only)
Allowed to schedule up to 1 appointment every month for routine check-ins, medication management, and brief touchpoints—
Messaging access with provider, designed for continuity and convenience between visits.
Monthly medication refills (cost does NOT include price of medication).
You can still use your insurance at the pharmacy for your medication.
