Thank you for your interest in care with Integrative Mental Health of the Midwest. Before your first appointment can be scheduled, all new patients must complete an initial new patient appointment request and once approved, the required intake, consent and insurance paperwork will be sent.
This helps our team review your information, confirm that the required documentation has been received, and provide appropriate next steps for scheduling.
Completing this request does not establish a provider-patient relationship.

Your date of birth
Your state of residence
The type of care you are seeking
Your insurance carrier, if applicable
A valid email address where you can receive next-step instructions
After you submit this request, you will receive instructions for completing the full intake packet. Scheduling instructions will be provided only after all required paperwork has been submitted and reviewed.
